Should Schools Ban Peanuts? What the Evidence-Based Guidelines Actually Recommend

The Sign on the Door Is Not the Policy
Plenty of parents choose a school partly because of a laminated notice at the front entrance announcing a nut-free building. It reads like a promise. Behind it sits an assumption worth testing: that removing one allergen from the premises is the thing keeping an allergic child safe.
The international practice guidelines on food allergy in schools say otherwise, and they say it plainly. A blanket site-wide ban is not the recommended approach. What the evidence supports instead is almost invisible from the parking lot: trained staff, stocked epinephrine, supervised eating, and a written plan for the specific child. That is uncomfortable news for any parent who fought hard for a nut-free classroom, and it is also the strongest argument you have when a school points at its sign and considers the job finished.
What the Guidelines Actually Say
The guidelines come from an international panel led by researchers at McMaster University, published in the Journal of Allergy and Clinical Immunology in May 2021. Susan Waserman, an allergist and professor of medicine at McMaster, chaired a group of 22 people (health care professionals, school administrators, and parents of children with and without food allergy) supported by six researchers with methodology expertise. They carry endorsements from the Allergy and Asthma Network, the American Academy of Allergy, Asthma and Immunology, the American College of Allergy, Asthma and Immunology, the Canadian Society of Allergy and Clinical Immunology, and the World Allergy Organization. That is close to a clean sweep of the field.
Two recommendations are straightforward. Child care and school personnel should be trained to prevent, recognize, and respond to allergic reactions. And sites should stock unassigned epinephrine auto-injectors, meaning devices held for general use rather than prescribed to one named student.
The third is the one that unsettles people. The panel recommends against site-wide food prohibitions such as nut-free schools, and against allergen-restricted zones such as milk-free tables, outside of limited special circumstances.
One honest caveat, because the panel led with it: the systematic review underpinning these recommendations found a shortage of high-quality evidence, so every recommendation is graded conditional. That is not the same as “just an opinion.” It means the direction of the evidence is clear enough to act on while staying open to revision as better studies arrive.
The American Academy of Pediatrics landed in the same place in its December 2025 clinical report, “Management of Food Allergy in Schools,” which replaced guidance dating from 2010. Its framing is that schools need strategies to reduce the chance of an allergen being eaten and procedures to recognize and treat a reaction when prevention fails. Both halves, not just the first.
Why Bans Do Not Deliver What They Promise
The most-cited evidence here is a 2017 analysis of Massachusetts public schools: 2,223 schools, 1,116,667 students, five years of data from 2006 to 2011, using epinephrine administration as the outcome. That is a sensible proxy, since it captures reactions serious enough that an adult reached for a device.
The result was flat. Schools that banned peanuts brought from home, banned peanuts from what they served, or ran peanut-free classrooms showed no lower rate of epinephrine administration than schools without those policies. No policy of any kind was associated with a complete absence of allergic reactions, and schools that called themselves peanut-free still reported reactions to nuts.
The mechanics of the failure are not mysterious.
- A ban is a rule about hundreds of families’ grocery shopping. It only works if every household reads every label correctly, every day, forever. Enforcement lives outside the building.
- “Peanut-free” means different things to different people. A parent packing a granola bar carrying a “may contain” advisory has usually decided they complied. A “free from” claim on a package describes a recipe, not a shared production line, and never guarantees zero cross-contact.
- A peanut ban covers one allergen. Milk, egg, wheat, soy, sesame, and fish walk through the door untouched. Milk is the most common childhood allergen of all, which is one reason a complete picture of dairy-free living matters as much as the peanut conversation.
- Belief in the ban erodes the habits that actually work. This is the false-security problem the guidelines are circling. When everyone assumes the allergen is gone, adults stop reading labels at the class party, the field trip snack goes unchecked, and the auto-injector ends up locked in a front-office drawer three corridors from the cafeteria.

The One Restriction That Did Show a Signal
The same Massachusetts study found a single exception, and it deserves to be reported rather than buried. Schools with peanut-free tables had lower epinephrine administration rates than schools without: 0.2 per 10,000 students against 0.6, a statistically significant gap.
The contrast is instructive. A site-wide ban is broad, unenforceable, and aimed at the whole population. A designated table is narrow, supervised, and aimed at the exact moment risk peaks, which is when food is open and children are eating.
This does sit in tension with the guidelines’ wording on allergen-restricted zones, which is partly why the panel carved out “limited special circumstances” (child care settings with infants and toddlers who cannot self-manage are the clearest example). It also carries a cost the guidelines take seriously. Children with food allergy face documented rates of allergy-related bullying, and a table that isolates one child is a poor trade. The workable version is an allergen-aware table where friends with safe lunches sit too, chosen by the family rather than imposed on them.
What Casual Contact Actually Does
Much of the emotional weight behind bans comes from a belief that proximity itself is lethal. The research on this is more reassuring than most parents expect.
In a double-blind, placebo-controlled study published in 2003, 30 children with significant peanut allergy (median peanut IgE above 100 kIU/L, many with a reported history of contact or inhalation reactions) were exposed to peanut butter two ways: pressed against intact skin for one minute, and set 12 inches from the face for 10 minutes of breathing. None had a systemic or respiratory reaction. A handful developed redness, itching, or a wheal at the skin contact site and nothing more. The authors concluded with 96% confidence that at least 90% of highly sensitive peanut-allergic children would not have a systemic reaction from casual exposure.
Read that carefully, because it is easy to over-apply. It says casual contact is not the main danger. It does not say nobody ever reacts, it says nothing about eating the allergen, and large-scale cooking that aerosolizes protein is a different exposure entirely. Ingestion remains the thing that kills people, which is precisely why the guidelines redirect effort toward the mouth rather than the room.
A 2004 study by the same research group measured what removes peanut protein from hands and surfaces. Liquid soap, bar soap, and commercial wipes left the major peanut allergen Ara h 1 undetectable. Plain water left residue on 3 of 12 hands, and hand sanitizer left it on 6 of 12. Most common cleaners removed it from tabletops, though dish soap left residue on 4 of 12 samples. Handwashing with soap before and after eating is one of the cheapest, highest-yield interventions a school can adopt, and sanitizer is not a substitute. Sanitizer kills germs. It does not lift protein off skin.

What the Guidelines Recommend Instead
Strip out the ban and the list that remains is more demanding, not less.
- Train every adult who supervises children, not just the school nurse, to prevent, recognize, and respond to a reaction. Anaphylaxis has been estimated to occur in roughly 1 in 15 schools each year, and the adult present is rarely a clinician.
- Stock unassigned epinephrine on site and know where it is.
- Supervise snack and meal times. Adult eyes at the table are the intervention the guidelines name first among common-sense measures.
- Keep allergens out of curriculum and activities. Counting with peanuts, egg carton crafts, milk carton science projects, bake sales, and unvetted field trip snacks are all avoidable exposures.
- Require handwashing with soap before and after eating.
- Write an individual plan for each diagnosed student, with the allergens, symptoms, and treatment steps in plain language.
- Make no food sharing the default norm, which protects every allergic child rather than one.
- Run an anti-bullying policy that names food allergy, since exclusion and teasing are part of the risk picture.
Stock Epinephrine Is the Load-Bearing Piece
If a school does one thing from that list, this is it. All 50 states and the District of Columbia now permit schools to hold undesignated epinephrine, but far fewer require it: the Asthma and Allergy Foundation of America’s 2025 State Honor Roll counted only 10 states that mandate stocking. Permission is not possession.
The legal picture is also moving. Around 26 states have shifted their statutes to broader language about “epinephrine delivery systems” or “devices” rather than auto-injectors specifically, which opens the door to nasal epinephrine, and more of those changes take effect through 2026. How to Judge Your School Past the Sign
Ask these, and treat a vague answer as a no.
- Does the school stock unassigned epinephrine? How many devices, where are they kept, are they unlocked, and when do they expire?
- Who is trained to recognize anaphylaxis and use a device? All staff, or only the nurse? What happens on a day the nurse is out?
- Does epinephrine travel on field trips, on the bus, and to after-school programs?
- Is an adult assigned to supervise eating in the cafeteria and at classroom parties?
- Is handwashing with soap built into the routine before and after eating, or is sanitizer the default?
- How are allergens handled in lessons, crafts, celebrations, and fundraisers?
- Does my child have a current written allergy action plan on file, and has every adult who supervises them read it?
- Does the anti-bullying policy explicitly cover food allergy?
A school that answers all eight well is safer than a nut-free school that answers none of them, whatever the front door says.
The Bottom Line
The nut-free sign is a comfort object. The evidence says it does not lower the rate at which children need emergency epinephrine, while the measures that plainly help are unglamorous: trained adults, a device within reach, supervision at the table, soap, and a plan written for your specific child.
